There are three distinct stages:

Grade 1: The nipple is located at the level of the inframammary fold.

Grade 2: The nipple is located 1–3 cm below the inframammary fold.

Grade 3: The nipple is located more than 3 cm below the inframammary fold.

There are various causes of sagging breasts.

• Age: The volume of glandular tissue decreases with age, while muscles and skin stretch.
• Pregnancy and Breastfeeding: The breasts enlarge during pregnancy and fill with breast milk after childbirth; the mammary glands swell. Once breastfeeding is over, the breasts lose volume, but the skin does not retract. The breasts become saggy.
• Significant weight loss or weight fluctuations: If a woman loses a lot of weight quickly, her breast volume decreases, but the connective tissue does not shrink at the same rate—the skin sags, so the breasts sag.
